COMPETITIVE REGISTRATION

Needing a home association to register your competitive team? Carrollton Farmers Branch Soccer Association has registered many club and independent teams in the DFW area now for over 4 years. Registration will open on July 1, 2011. For rates or any questions, please contact Nicole James.

Competitive Info and Forms:

Passing background check for all members of coaching staff (must be completed before adding to roster)

USYSA membership form for each player

Birth certificates will need to be verified, copies will not be kept

RECREATIONAL REGISTRATION

Fall 2011/Spring 2012 Soccer Season -
Registration Dates: May 1, 2011 – July 15, 2011

* $100 (U4 only - New Age Group this year!) Soccer registration for Fall (8 games - weather permitting) & Spring (8 games - weather permitting)

* $120 (U5 - U19) Soccer registration for Fall (10 games - weather permitting) & Spring (10 games – weather permitting)

* $70 Head Coach (of the team your child plays and you head coach) for full year. Only one discount per child.

* $70 for 3rd sibling or more in CFBSA (excluding select) for full year. Must set up Family Account to receive discount.

Registration Fees: Make check/money order payable to CFBSA. We also accept VISA/MC/DISCOVER! NO CASH ACCEPTED, NO EXCEPTIONS! A $50 NSF fee will be charged for all returned checks.

Refunds: A 50% refund will be issued to any player who decides, prior to the first game of the Fall season, not to play on a CFBSA team to which he/she was assigned with written request. No refunds after the Fall season starts, no exceptions.

Late Registration: Any registration received after July 16, 2011 will incur a $25 late fee and no guarantee of placement on any team. Players registering late must print the registration form (see forms below) and either mail or drop off at CFBSA office with full registration fee plus late fee.

All registrations must include the following information to be accepted:
1) Registration Form - filled out and signed
2) Association Creed - signed
3) Birth Certificate (copy only for new players)
4) Full and correct fee
