REGISTRATION DATES:
May 01, 2010 - July 23, 2010
FEES:
- $120.00 (U5-U19) Soccer registration for 1
full soccer year (10 games in Fall /10 games in
Spring - weather permitting)

- $100.00 (U4) Soccer registration for 1 full
soccer year (8 games in Fall /8 games in Spring - weather permitting)
- $70.00 Head Coach (of the team your child
plays and you coach) for a full year
- $70.00 Board Member/ League Director’s child
for a full year
- $70.00 for 3rd sibling or more in CFBSA
(excluding select) for a full year
No cash will be accepted, no exceptions!! Make
check/money order payable to CFBSA. We also
accept VISA/MC/DISCOVER! A $50.00 NSF fee will
be charged for all returned checks.
A 50% refund will be issued to any player who
decides, prior to the first fall game of the
season, not to play on a CFBSA team to which
he/she was assigned with written request. No
refunds after the fall season starts, no
exceptions! Please keep in mind, when you
register your child beginning in May, you’re
registering them for the full soccer year, which
includes the fall and spring soccer season.
Any registration received between July 24,
2010-August 28, 2010 will incur a $25.00 late
fee.
Any registration received after August 28,
2010 will incur a $50.00 late fee.
Before any new registration can be accepted
you must submit
the following information:
1) Full and Correct Fee
2) Birth Certificate (new player only)
3) Registration Form
4) Signed copy of Association Creed

Registration Information
Fall 2010 competitive registration:
1. Register your team @

2. Update all information in your Got
Soccer account, then call me for a time to meet
to hand over copies for me to keep. At that
time, I will hand you your printed rosters and
ID cards.
3. Call me to set up a time to meet, give
me all your paperwork to process in your Got
Soccer account. I will then create your rosters
and ID cards and mail them to you.
4. Mail (or hand deliver) all your paper
work, once received; then I'll produce your
rosters and ID cards and mail them to you within
48 hours.
The information I need from each of you is
the following:
- NTSSA/USYS Competitive Membership Player
Form for every player
- Proof of Age-Birth Certificate-Passport
or Drivers License for new players only,
these will be returned to you
- Medical release forms notarized and
signed these will be returned to you
- NTSSA/USYS Non-Recreation-Competitive
Team Registration Roster
- Copy of Team/Club Fact Sheet Signature
